MYLB Registration:

Step 1:  Download the Registration Form fill it out.  Download Registration form

Step 2:  Save the registration form you filled out (click on the save icon or if you are using adobe acrobat or apple preview click file save as and choose a name for your filled out form.

Step 3:  Click on the Browse button below and locate the file you saved.

Step 4:  Click submit and proceed to payment for registration on the right of the page via paypal. (Cost for Upper & Lower Clinics are $110. All other divisions is $120)
